Is a Cracked Porcelain Crown a Dental Emergency?

I had several porcelain crowns placed on my front teeth. They’ve served me well for over fifteen years. One has always had a minor defect you couldn’t see just by looking. My dentist told me about it and offered to do a different crown, but she did say that even our natural teeth have minor defects and it shouldn’t be a problem. I appreciated her honesty and her willingness to make an entire new one. I didn’t feel that was necessary and their longevity has born that out. Today, however, I noticed I can both see and feel the defect. I’m sure it needs to be replaced at this point, I’m just wondering if it is a dental emergency or I can wait. The original dentist has since retired and her replacement and I are just getting to know one another so I don’t yet have the same confidence in her.

Lindsey

Dear Lindsey,

A Dental Crown being Placed

It sounds like you and your old dentist had a wonderful doctor-patient relationship. It’s certainly hard to lose a trusted caregiver. Hopefully, her replacement will end up being equally honest and skilled. While this change in your dental crown isn’t necessarily a dental emergency, I don’t want you to get too comfortable and let this slide. You do need to be seen.

The fact that you can now see and feel the defect tells me it’s cracked. It will at some point completely break. Those type of things inevitably happen at the worst possible moments, like when you’re on vacation or out to dinner. It’s much better for you to get this dealt with well before it can become an emergency issue. Plus, it will give you peace of mind knowing it’s no longer at risk of breaking.

Who Should Replace Your Dental Crown?

You’re not familiar with your new dentist yet, so before you allow her to replace your porcelain crown, you will need to research her cosmetic skill. At the very least check out her smile gallery. It’s a collection of before and after pictures of cosmetic work she’s done. You can click here to look at Dr. Burba’s work to get an idea of the type of quality you want. These are front teeth so you want them to be beautiful when you smile.

Some dentists would try to persuade you to replace all the crowns so as to make them easier to match. That’s usually a sign they’re not top-notch cosmetic dentists. The crowns are aging, so if you want to go ahead and replace all of them, that’s fine. But, you mentioned they’re still in good shape. In that case, you really only have to replace the broken one.

Replacing a single crown to your remaining teeth takes an expert cosmetic dentist. I’d suggest you look for a dentist who’s reached accreditation level with the American Academy of Cosmetic Dentistry. AACD accredited dentists are experts in their field both in technical skill and artistry. They’ll match your crown perfectly.

How to Find the Best Dentist When You Move

We’re moving across the country. We’ve had a fantastic dentist all of our lives. It’s sad to me that we have to leave the clinic. I’m pretty sure we’ve had the best dentist in our state. Is there an easy way to locate the best dentist in our new state?

Emily P.

Dear Emily,

Best Dentist in Boston

I’ll give you some tips to narrow down your dentist choices. The first thing you need to do is decide what services are important to you. For instance, if you’ll want someone who focuses only on general dental services you’ll go one route. But, if you are interested in cosmetic dental services there will be additional things to look for.

I’m going to go on the assumption you’re looking for a good cosmetic dentist because those are harder to find. There is no recognized specialty for cosmetic dentistry so every cosmetic dentist is just a general dentist who offers cosmetic procedures. As with any art based skill, there are a large variety of skills among the artists.

Because it’s extremely difficult for patients to know ahead of time which dentists are skilled at creating beautiful smiles and which are really bad at it, the American Academy of Cosmetic Dentistry (AACD) developed an accreditation program. Those cosmetic dentists which reach accreditation are among the most skilled, artistic dentists in the world. If getting a smile makeover is important to you, you’ll want an AACD accredited dentist.

Now, you likely fell in love with your dentist because of his personality. He showed interest and care about your family. It won’t do you any good if your dentist is artistic, but a total jerk. So, once you’ve found an AACD accredited dentist in your area, look at their smile gallery to be certain you like the results they get. You’ll also want to make sure they have some form of beautiful smile guarantee. Then, go onto sites like Google Review or Yelp! and read reviews patients have given them. You want to make sure they’re good with patients.

If you take these steps, you’re very likely to find at least one of the best dentists in your new area. If cosmetic work isn’t important to you, then just look at reviews to make sure they’re friendly and do work that holds up.

Want a dazzling smile

I’m going to have some cosmetic work done and I’m trying to find the best dentist in to do it.  My dentist suggested veneers and showed me his before and after pictures of cases he’s done.

I wasn’t that impressed with his pictures and wanted my smile to be more of a dramatic dazzle kind of thing. I want my smile to light up a room. How do I find a dentist who can do that?

Olivia R. – Virginia

Olivia,

You’re wise to look for a different dentist to do your cosmetic work. There are many well meaning family dentists, who are awesome at general dentistry, but just don’t have the artistic training for really gorgeous cosmetic work.

It’s a good idea to look for a smile gallery on each candidate’s website, so you can get a feel for whether his cosmetic work is something you’d be proud to show off. But, there are other things to look for as well.

Have they done significant post-graduate training in cosmetic work? Do they guarantee their work? Many good cosmetic dentists have something akin to a  “Beautiful Smile Guarantee,” which simply means that if you aren’t happy with your smile after the cosmetic work is done, the doctor will be glad to correct it. The procedures not considered finished until you’re thrilled.

There is a definitive way to be certain you’re getting the best possible dentist for your cosmetic work. If I were in your place, I’d want a dentist who was accredited with the American Academy of Cosmetic Dentistry (AACD).  There is a difference between membership and accreditation, so make sure he’s accredited.

These are among the top 1-3% of cosmetic dentists, not just in the country, but in the world.
